This full-time role offers flexible scheduling and the chance to build lasting patient relationships. You will perform visits, OASIS assessments, educate families, and coordinate with the care team to ensure every detail supports positive outcomes.

Qualifications

• Active RN license in Oklahoma.
• Home health experience preferred; strong assessment and critical thinking skills.

Responsibilities

• Develop and adjust the plan of care for patients in collaboration with the care team.
• Complete visits and OASIS assessments with comprehensive documentation.
• Coordinate with the care team to ensure nothing falls through the cracks.
• Educate patients and families on diagnosis, medication, and home safety.
• Advocate for patients to drive real outcomes.
