Interim HealthCare of Oklahoma City is seeking an experienced Home Health RN Case Manager in Oklahoma City. You will join a full care team and shape the plan of care for your patients, not just complete tasks. The role offers autonomy, meaningful patient relationships, and a voice in care decisions.

We value strong assessment and critical thinking, with flexible pay structures: either salary or per-visit. Full-time, Monday–Friday with day-to-day flexibility built around patient needs.

Qualifications

• Active RN license in Oklahoma.
• Experience in home health is ideal; strong assessment and critical thinking skills required.
• CPR certification required.

Responsibilities

• Develop and adjust the plan of care for patients with the care team (LPNs, therapists, physicians).
• Complete visits, OASIS assessments, and documentation with a patient-centered view.
• Coordinate with the care team to prevent gaps in care.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients to drive meaningful outcomes.
