Job summary

Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager in Oklahoma City. You will develop and adjust the plan of care with a collaborative team, including LPNs, therapists, and physicians, using real clinical judgment rather than simply checking boxes.

Schedule is primarily Monday–Friday with two weekends per month for full-time; part-time involves one weekend per month. Earn either salary or per-visit pay, plus benefits, with weekly pay and company-matched 401(k).

Qualifications

• Active RN license in Oklahoma.
• Home health experience and strong assessment/critical thinking are valued.
• CPR certification is required.

Responsibilities

• Develop and adjust the plan of care with the care team.
• Complete visits, OASIS assessments, and documentation.
• Coordinate with the care team to prevent gaps in care.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients to drive real outcomes.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What You'll Earn

We offer flexible pay structures for this role, either salary or per-visit, whichever fits you best:

• Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
• Salary option available for the right candidate

What We're Looking For

• Active RN license in Oklahoma
• Home health experience is ideal, but we're also looking for experienced nurses from any clinical background who are ready for a change and bring strong assessment and critical thinking skills
