Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager in Oklahoma City to bring real clinical judgment to patient care. You will work with LPNs and support staff to shape the plan of care and advocate for patients.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What You'll Earn

We offer flexible pay structures for this role, either salary or per-visit, whichever fits you best:

• Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
• Salary option available for the right candidate

• Complete visits, OASIS assessments, and documentation with an eye toward the full patient picture, not just the task in front of you

• Advocate for your patients and help drive real outcomes

What We're Looking For

• Active RN license in Oklahoma
• Home health experience is ideal, but we're also looking for experienced nurses from any clinical background who are ready for a change and bring strong assessment and critical thinking skills
• CPR certificationA nurse who wants autonomy, real patient relationships, and a genuine say in the plan of care
