Qualifications

• Active RN license in Oklahoma required.
• Home health experience preferred; strong assessment and critical thinking encouraged.
• CPR certification required; autonomy and patient-centered care mindset.

Responsibilities

• Develop and adjust care plans with the care team.
• Complete visits and OASIS assessments and documentation.
• Coordinate with the care team to ensure no gaps in care.
• Educate patients and families on diagnoses, medications, and home safety.
• Advocate for patients to drive real outcomes.
