You will complete visits, assessments, and documentation with a focus on the whole patient, not just tasks. The role emphasizes autonomy in patient care and active coordination with the care team to prevent gaps.

Qualifications

• Active RN license in Oklahoma.
• Experience in home health or similar clinical setting preferred.
• Strong assessment and critical thinking skills.
• Ability to work autonomously and coordinate with a care team.

Responsibilities

• Develop and adjust the plan of care with the patient and care team.
• Complete visits, OASIS assessments, and documentation.
• Coordinate with LPNs, therapists, and physicians to ensure seamless care.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients and drive real outcomes.
