Job summary

21st Century Home Health Services (21HHS) is seeking a Speech Therapist/Language Pathologist for Santa Clara County (San Jose area). You will deliver skilled services in patients’ homes, collaborate with Case Managers, and contribute to high-quality outcomes with an emphasis on compassionate care.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and human behavior as it relates to health care and illness/injury.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Home health care experience preferred.
• Experience with high acuity patients a plus.
• Experience working with an interdisciplinary team highly desired.
• Case management fundamentals desired.
• Self-directed and organized.
• Empathetic and excellent interpersonal skills.
• Leadership skills.
• Strong communication and critical thinking abilities.
• Prior to scheduling an in-person interview, complete a computer literacy and competency assessment.

Responsibilities

• Provide skilled speech therapy to assist patients with breathing, speaking, and swallowing.
• Evaluate and treat speech, language, cognitive and fluency disorders affecting communication.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
