Job summary

21st Century Home Health Services in the San Mateo County area seeks a Speech Therapist/Language Pathologist to provide in-home services in partnership with the Case Manager. You will evaluate and treat speech, language, cognitive and swallowing disorders and serve as an educator and advocate within the interdisciplinary care team.

You will collaborate closely with Case Manager and other clinicians to deliver high-quality, patient-centered care in patients' homes across the territory.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR certification.
• Valid CA Driver's License.
• Knowledge of culture's impact on care.
• State and federal homecare guidelines preferred.
• Experience in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Self-Directed, empathetic and critical thinker.
• Outstanding interpersonal skills and leadership.
• Organized and proactive.
• Pre-scheduling assessment competency required before interviewing.

Responsibilities

• Provide skilled speech, language, cognitive and swallowing therapy.
• Evaluate and treat communication disorders and related deficits.
