Job summary

21st Century Home Health Services is seeking a Speech Therapist/Language Pathologist to join our team serving Solano County (San Rafael area) and surrounding counties. You will evaluate and treat speech, language, cognitive, and swallowing disorders in home-bound patients as part of an interdisciplinary care team.

Responsibilities include education and advocacy, flexible scheduling, and a focus on high-quality patient care. Travel to patient homes is required; a CA driver’s license is essential.

Qualifications

• Current unrestricted CA Speech Therapist License is required.
• BLS/CPR Certification required.
• Valid CA Driver's License.
• Experience in home health care and interdisciplinary teams preferred.

Responsibilities

• Provide skilled ST services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive, and fluency disorders.
• Collaborate with Case Manager and interdisciplinary care team to educate patients and advocate for them.
