21st Century Home Health Services (21HHS) is seeking a Home Health Speech Therapist/Language Pathologist to join our field-based team serving patients across the Bay Area.

In partnership with the Case Manager, you will evaluate and treat speech, language, cognitive and swallowing disorders, and educate patients and families within the home setting.

You will collaborate with an interdisciplinary care team to support compassionate, high-quality care as we expand into Marin and Sonoma counties.

Responsibilities

• Provide skilled services to assist patients with breathing, speaking, and swallowing.
• Evaluate and treat speech, language, cognitive and fluency disorders affecting communication.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
