Job summary
21st Century Home Health Services is seeking a Speech Therapist/Language Pathologist (ST) to serve Sacramento County patients in home health. You will evaluate and treat speech, language, cognitive and swallowing disorders within an interdisciplinary team, partnering with Case Managers to tailor care plans.
Ideal candidates hold a CA State Speech Therapist license, a current BLS/CPR certification, and a CA driver's license.
Qualifications
- Current unrestricted CA Speech Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver's License.
- Knowledge of development and behavior in illness/injury; culture impacts care.
- Regulatory guidelines for home health preferred.
- Home health care experience preferred.
- Experience with high acuity patients a plus.
- Experience with interdisciplinary teams desired.
- Case management fundamentals desirable.
- Self-directed, empathetic, and organized.
- Strong interpersonal and leadership skills.
- Crucial: complete computer literacy/competency assessment before interview.
Responsibilities
- Provide skilled services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive and fluency disorders relevant to communication.
- Serve as educator, patient advocate and member of the interdisciplinary care team.
- Travel within Sacramento County to visit patients at home.
