Job summary

21st Century Home Health Services (21HHS) seeks a Speech Therapist to provide skilled speech-language pathology services to patients in their homes as part of an interdisciplinary team in the San Francisco Bay Area.

You will work with case managers to evaluate, diagnose, and treat communication, cognitive, and swallowing disorders, while educating patients and caregivers and promoting optimal outcomes in a growing home health program.

Responsibilities

• In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
• You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
