Job summary

21st Century Home Health Services (21HHS) is seeking a Speech Therapist/Language Pathologist to provide in-home skilled services across Santa Cruz County, with a focus on Watsonville. You will evaluate and treat speech, language, cognitive, and swallowing disorders, working closely with Case Management and a care team to educate patients and advocate for high-quality care.

Qualifications

• CA Speech Therapist License is required.
• BLS/CPR certification is required.
• Valid CA Driver's License is required.
• Experience in home health care is highly preferred.
• Knowledge of state homecare regulatory guidelines is preferred.

Responsibilities

• Provide skilled speech therapy services in patients' homes.
• Evaluate and treat speech, language, cognitive, and swallowing disorders.
• Collaborate with the interdisciplinary care team and Case Manager to optimize patient outcomes.
