You will assess, triage, initiate interventions, and coordinate with interdisciplinary teams to prevent hospitalizations and ensure timely care.

Qualifications

• Graduate of an accredited school of Nursing.
• RN license current/valid.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Provide autonomous urgent and emergent care to PACE participants.
• Assess and triage participants with acute or unstable conditions.
• Initiate evidence-based interventions and escalate to providers as needed.
