Job summary

myLaurel is recruiting a Nurse Practitioner for acute care in the home. The role emphasizes autonomy, high-acuity home visits, and comprehensive patient management. You will perform hands-on assessments, IV access, and medications with independence, reporting to the Field Provider Manager.

Flexible evenings, weekends, and holidays are required. Ideal candidates hold WA NP licensure (ARNP/PA-C), 2+ years acute care experience, and BLS; ACLS is a plus.

Qualifications

• WA state NP license required.
• 2+ years acute care experience in ED, inpatient, or LTACH.
• Ability to perform IV access and procedures independently.
• ACLS preferred; BLS required.

Responsibilities

• Perform home-based assessments and interventions.
• Order and interpret tests and coordinate holistic care plans.
• Participate in quality initiatives to reduce readmissions and improve outcomes.
