Pride Health seeks a Clinical Care Registered Nurse to provide direct nursing care to PACE participants in both clinic and home-based environments. The RN will manage acute and chronic conditions, perform assessments, administer medications, provide wound care, and collaborate with the interdisciplinary team to support community-based living.

Responsibilities include home visits, care plan development, and nursing interventions as needed.

Qualifications

• Active RN license is required.
• ADN required; BSN preferred.
• Minimum 1 year of experience with frail, elderly, or long-term care patients.
• Current CPR/BLS certification or ability to obtain via employer.

Responsabilités

• Provide direct nursing care to frail and elderly patients in home and clinical settings.
• Perform comprehensive assessments and contribute to individualized care plans.
• Monitor acute and chronic conditions in collaboration with PCPs.
• Triage emergent situations and determine appropriate nursing interventions.
• Conduct regular home visits and administer wound care and other skilled nursing treatments.
• Administer medications, immunizations, and required procedures; perform lab draws.
