United Health Centers is recruiting a PACE Registered Nurse (RN) to support our new PACE site, delivering clinical care and intake assessments to help seniors stay at home.

Under the direction of primary care, you will assess, coordinate, and provide treatment, participate in the interdisciplinary team, and supervise medication administration and wound care. CA RN license and ACLS certification required; bilingual preferred.

Formación

• Licensed RN in CA with an active license.
• ACLS certification is required.
• 1 year of RN experience; elder care preferred.
• Bilingual English/Spanish preferred.

Responsabilidades

• Provide care to PACE participants within licensure scope and PACE model.
• Administer medications and manage medication systems.
• Triage, document, and coordinate with the IDT for care plans.
• Wound care and daily nursing tasks in clinic.
• Maintain participant safety and assist in emergencies.
