Job summary

CenterWell Senior Primary Care in The Villages, FL seeks an autonomous Wound Care Nurse Practitioner to lead wound, ostomy, and continence care. You will assess complex wounds, oversee department care delivery, and collaborate with physicians to optimize patient outcomes.

Expect leadership responsibilities and advanced practice autonomy with ongoing staff education. The role requires Florida APRN with Autonomous designation, 3,000+ clinical hours, and wound care certification within 6 months.

Qualifications

• Active Florida APRN license with Autonomous designation.
• 3,000 clinical hours under physician supervision within the last 5 years.
• Three graduate‑level hours in differential diagnosis within the last 5 years.
• Board Certification in Family or Adult.
• Wound Care certification required within 6 months of hire.
• 3–5 years of clinical wound/ostomy experience.
• Current CPR/BLS or ACLS certification.
• Immunizations as required for healthcare work.

Responsibilities

• Assess, treat and manage diverse wounds to promote healing and improved outcomes.
• Provide clinician oversight of wound care department care delivery.
• Perform comprehensive wound assessments and develop plans of care with advanced dressing and treatment modalities.
• Refer patients to vascular/plastic surgery or podiatry as needed.
• Lead pressure injury prevention initiatives with staff guidance and protocols.
• Conduct bedside procedures such as debridement, biopsies, I&D, and skin substitute applications.
• Order targeted pharmacologic treatments and diagnostic tests.
• Provide ostomy care services and postoperative management.
• Participate in multidisciplinary case reviews to ensure quality of care.
• Serve as clinical resource and mentor for staff on wound care and ostomy.
• Educate patients and families about medications, treatments, and health promotion.
