Job summary

Prisma Health's SeniorCare PACE program seeks an autonomous RN to deliver advanced clinical care across clinic and home settings. You will assess, triage, treat, and escalate as needed, acting as a frontline responder for urgent conditions.

Strong wound care and IV therapy skills, plus independent decision‑making, are essential. Education includes RN licensure with a BSN preferred; one year with frail or elderly populations is desirable.

Qualifications

• Graduate of an accredited nursing program and state RN licensure.
• Bachelor's degree in Nursing preferred.
• One year experience with frail or elderly population.

Responsibilities

• Deliver advanced, time sensitive clinical care to frail PACE participants in clinic and home settings.
• Perform real‑time assessments, triage and initiate evidence‑based interventions per protocol.
• Coordinate with interdisciplinary team for urgent care and escalation when needed.
• Document care encounters with rationale and outcomes; monitor and adjust plans.
