Job summary

Humana, Inc. and CenterWell Senior Primary Care are seeking a Wound Care Nurse Practitioner to act as an autonomous APRN, delivering expert wound, ostomy, and continence care. You will lead clinical care, educate staff and patients, and collaborate across specialties to improve outcomes.

The role emphasizes advanced wound management, independent practice under Section 464.0123, and participation in quality initiatives within a comprehensive care model.

Qualifications

• Active Florida APRN license with Autonomous APRN designation.
• Board Certification in Family or Adult required or within 6 months of hire.
• Wound care certification required or obtained within 6 months of hire.
• Minimum 3–5 years of clinical wound/ostomy care experience.
• Current CPR/BLS or ACLS certification.

Responsibilities

• Assess, treat and manage diverse wounds to improve patient outcomes.
• Provide oversight of Wound Care Department care delivery.
• Perform initial wound assessments and develop care plans.
• Refer patients to vascular/plastic surgery or podiatry when needed.
• Lead pressure injury prevention initiatives and protocols.
• Conduct bedside procedures such as debridement and biopsies.
• Order targeted pharmacologic treatments and diagnostic tests.
• Coordinate ostomy care services with the team.
• Participate in multidisciplinary case reviews to ensure quality.
• Educate patients and families on care plans and treatments.
