Job summary

Blue Ridge Care is seeking a Registered Nurse (RN) for our PACE program to provide direct in-home nursing care to older adults in Virginia. You will work with an interdisciplinary team to support independence and safety in participants' homes and community settings.

The role emphasizes autonomous clinical assessment, care planning, and caregiver education, with opportunities to collaborate across teams and improve life quality for seniors in the PACE program.

Qualifications

• Current RN licensure in Virginia.
• BLS/CPR certification is required.
• Minimum 1 year with frail/elderly population preferred.
• Strong clinical assessment and critical thinking.
• Experience in home health, geriatrics, hospice, or community care is preferred.
• Medical clearance for communicable diseases and up-to-date immunizations.

Responsibilities

• Conduct in-home nursing assessments and evaluations.
• Perform skilled nursing visits: medications, wound care, injections, phlebotomy, chronic disease monitoring.
• Communicate changes with the IDT and providers.
• Develop and monitor individualized care plans with IDT.
• Educate participants and caregivers on disease management and safety.
