Job summary

Iconic Care is seeking a physiatrist to provide on-site, autonomous care across multiple facilities. You will manage 25+ patients daily, perform targeted injections, and collaborate with facility teams to optimize rehabilitation participation and outcomes.

The role offers a flexible 5-day schedule, no nights or on-call shifts, and competitive production-based compensation with benefits. Start date in 4–6 weeks post credentialing, full-time contract.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort performing peripheral joint injections (knees, shoulders, hips).
• Autonomy in independent practice with team collaboration.
• Strong communication and multidisciplinary collaboration skills.
• Familiarity with EHR charting.

Responsibilities

• Round at 1–3 SNFs or rehab centers 1–3 days per week per location.
• Deliver physiatry care to 25+ patients per day to aid therapy participation and recovery.
• Perform injections and medication management as needed.
• Serve as the on-site physiatrist with autonomy and consult as needed.
• Coordinate care and refer to other specialties when required.
