Job summary

Iconic Care is seeking a physician to serve as a full-time Physiatrist at multiple skilled nursing facilities. You will round 1–2 days per week at each location, delivering rehab-focused care to 28–30 patients daily, and lead on-site with autonomy while collaborating with facility teams.

Responsibilities include focused assessments, individualized treatment plans, injections (joint), Botox, dry needling, and medication management, plus coordinating care with other specialties.

Qualifications

• Current, unrestricted PA or NP license.
• DEA license or willing to obtain.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Passion for personalized, autonomous care and improving rehabilitation outcomes.
• Strong communication with multidisciplinary teams and familiarity with EHR.

Responsibilities

• Perform focused assessments and develop individualized treatment plans with facility teams to optimize therapy participation and recovery.
• Provide interventions such as joint injections, Botox, dry needling, and medication management based on patient needs.
• Serve as the sole physiatrist on-site with autonomy, while supported by a collaborative clinical team for consultation.
• Coordinate care and refer to other specialties for conditions beyond physiatry.
