Job summary

Iconic Care is seeking a licensed physiatrist to provide rehab-focused care in skilled nursing facilities. You will round 1–3 days per week at each site, delivering specialized physiatry care to 25+ patients daily who struggle to participate in therapy due to pain or functional limitations.

You will perform injections, manage medications, and coordinate with facility primary care teams. This role emphasizes autonomy with a collaborative clinical support structure and a flexible 5-day workweek.

Qualifications

• Current, unrestricted PA or NP license.
• DEA license or willingness to obtain.
• Nursing home or physiatry experience is a plus.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.

Responsibilities

• Rounding at 1–3 Skilled Nursing Facilities 1–3 days per week.
• Deliver specialized physiatry care to 25+ patients daily to improve therapy participation.
• Perform joint injections, Botox, dry needling, and med management as needed.
• Serve as sole on-site physiatrist with autonomy, supported by a clinical team.
• Coordinate care and refer to other specialties as needed.
