Job summary

Iconic Care Support Services in Bountiful, UT is seeking a Physiatry (PM&R) Nurse Practitioner or Physician Assistant to join full-time. You will deliver specialized rehab care across skilled nursing facilities with autonomy and collaboration.

Ramps-up to 27–30 patients daily, focused assessments, joint injections, Botox, dry needling, and medication management, with a competitive salary starting at $135,000+ per year plus benefits.

Qualifications

Current, unrestricted PA or NP license.
DEA licensure required or willing to obtain.
Nursing home and/or physiatry experience is a plus, but not required.
Comfort with peripheral joint injections (knees, shoulders, hips).
Strong communication and collaboration with multidisciplinary teams.
Familiarity with EHR charting.

Responsibilities

Perform focused assessments and develop individualized treatment plans with facility teams to optimize therapy participation.
Provide interventions such as joint injections, Botox, dry needling, and medication management.
Serve as the sole physiatrist on-site with autonomy, supported by a collaborative clinical team.
Coordinate care and referrals for issues beyond the scope of physiatry.
