Job summary
21st Century Home Health Services in Petaluma is seeking a Full-Time Occupational Therapist to serve patients in their homes. You will evaluate and address needs to help individuals live safely and productively at home, working with an interdisciplinary team.
You’ll enjoy flexible scheduling, territory focus, autonomy, and comprehensive benefits with a compensation package that includes base pay and incentive bonus. CA license, BLS/CPR, and driver’s license required.
Qualifications
- Current unrestricted CA Occupational Therapist License and active status.
- BLS/CPR certification required.
- Valid CA driver's license and clean driving record.
- Knowledge of home health care guidelines and resource networks.
- Experience with high-acuity patients and interdisciplinary teams is preferred.
Responsibilities
- Provide skilled OT services to evaluate and address home-based needs.
- Collaborate with Case Manager to promote safe, productive home living.
- Educate patients and families; advocate within the care team.
- Contribute to care planning and coordination across providers.
