Job summary

21st Century Home Health Services (21HHS) seeks a full-time Occupational Therapist to serve patients in their homes in the Sebastopol area. You will assess needs, create treatment plans, and deliver skilled OT services in a home health setting.

As an autonomous team member, you will coordinate with Case Managers and the interdisciplinary care team, educate patients and families, and support safety, independence, and quality of life in the home. CA OT license and BLS/CPR are required.

Qualifications

• Must hold current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local resources and community services.
• Home health care experience preferred; ability to work in an interdisciplinary team.

Responsibilities

• Evaluate and address the needs of individuals in their homes to promote safety and independence.
• Develop and implement skilled OT treatment plans and educate patients and families.
• Collaborate with Case Managers and the interdisciplinary care team to coordinate care.
