21st Century Home Health Services (21HHS) seeks a Full Time Occupational Therapist to serve patients in San Francisco with in-home visits. You will evaluate and address patient needs to help them live safely and productively at home, in partnership with a Case Manager and interdisciplinary team.

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.

Responsibilities

• Provide skilled OT services to evaluate/address needs of individuals in their homes.
• Collaborate with Case Manager and interdisciplinary care team.
• Address home safety, medication management, wheel chair seating/positioning, pain management, falls prevention.
