Job summary

21st Century Home Health Services seeks an Occupational Therapist (OT) to deliver in-home therapy for patients across Northern California. You will work within a defined territory and collaborate with a multidisciplinary team to create and follow individualized treatment plans.

Responsibilities include evaluating patients, documenting progress, and ensuring high quality care while adhering to regulatory guidelines.

Qualifications

• Must hold a current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources required.
• Experience in home health care preferred.

Responsibilities

• Provide occupational therapy in patients' homes within assigned territory.
• Collaborate with an interdisciplinary team to plan and implement care.
• Occasional coverage outside primary area as needed for patient care.
• Adhere to state and federal homecare regulatory guidelines.
