Rehab Without Walls Neuro Rehabilitation seeks a Physical Therapist to provide neuro-focused home and community care. You will conduct evaluations, develop individualized treatment plans, and work in a true one-on-one setting to advance mobility and independence.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program
• Current, unrestricted Physical Therapist license in the practicing state
• Current Basic Life Support or CPR certification
• Minimum of one year's work experience as a physical therapist (preferred)
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
• Communicates effectively in verbal and written interactions
• Ability to lift 50 pounds; patient handling up to 100 pounds with safety
• Duties require standing and walking for extended periods; various physical requirements

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly reassess effectiveness of treatment plans and attend staffing meetings and family conferences
• Perform discharge evaluations and make recommendations for home and community safety
• Communicate patient's needs and progress to the treatment team, physician, patient and family
• Collaborates with other disciplines to ensure coordination of care and improve outcomes
