Job summary

Rehab Without Walls Neuro Rehabilitation seeks a Physical Therapist to provide high-autonomy, in-home neuro rehabilitation across home and community settings. You will control your schedule and caseload, delivering true one-on-one neuro rehab to improve real-world function.

The role supports patients with stroke, TBI, Parkinson's, and other neurological conditions, focusing on mobility and independence with minimal productivity quotas.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support (BLS) or CPR certification as required by state regulations.
• At least 1 year of PT experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injuries.
• Strong communication skills (verbal and written).
• Ability to lift up to 100 pounds with safety.
• Duties may involve bending, reaching, and standing for extended periods.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess treatment effectiveness and participate in staff meetings and family conferences.
• Perform discharge evaluations and provide home safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordinated care and improved outcomes.
