Job summary

Rehab Without Walls Neuro Rehabilitation in Concord, NH is seeking a Physical Therapist (PT) specializing in neuro home and community settings. You will conduct evaluations, develop treatment plans, and deliver true one-on-one neuro rehab in clients' homes and local environments.

This role offers flexible scheduling, clinical autonomy, and a focus on meaningful functional outcomes rather than productivity quotas, with competitive hourly pay and reimbursement for drive time and mileage.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in the practicing state.
• Current Basic Life Support or CPR certification as required by state.
• Minimum one year of PT experience preferred.

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations and provide home safety recommendations
• Communicate patient needs and progress to the treatment team and family
• Collaborates with other disciplines to improve patient outcomes
