Job summary

Rehab Without Walls Neuro Rehabilitation in Augusta, ME seeks a Physical Therapist to deliver in-home and community-based neuro rehabilitation with high autonomy and 1:1 care. The PT will evaluate, treat, and re-assess patients recovering from neurological conditions, including stroke, TBI, and Parkinson's, to improve mobility and independence.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current unrestricted Physical Therapist license in the practicing state.
• Current CPR/BLS certification as required by state regulations.
• Minimum of 1 year of PT experience preferred.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure collaboration and enhance patient outcomes.
