Job summary

MPAC Healthcare is seeking an on-site Licensed Clinician to function with the autonomy of a private practice while integrated into a nursing facility team. You will manage your caseload, conduct initial assessments, and provide psychotherapy, collaborating closely with physicians, nurses, and social services.

You will work a 40-hour week with emphasis on clinical care, documentation, and facility communication.

Qualifications

• Have a master’s degree and a valid LCSW/LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of mental-health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams to ensure patients receive the best care.
• Ability to foster open communication with the facility team and Regional Clinical Manager.
• Ability to take initiative and communicate effectively in multiple settings.
• High level of organization to manage patient needs and documentation.
• Comfortable with the changing environment of a skilled nursing facility.

Responsibilities

• Work a 40-hour week with most time clinically focused and some time on documentation and facility correspondence.
• Be an on-site clinical provider working with residents and their families.
• Lead a team on interdisciplinary rounds addressing residents’ emotional and psychological needs.
• Own your caseload, schedule, and salary.
• Provide initial assessments and ongoing psychotherapy for mental health issues.
• Maintain documentation requirements and stay updated on Medicare and commercial plans.
