Job summary

Qualifications

• Master’s degree in counseling, social work, or related field.
• Licensed in state (LISW/LMFT/LMHC) for the applying state.
• Minimum two years of counseling/therapy experience.
• Experience in nursing home or hospital facility setting preferred.
• Ability to work in interdisciplinary teams and communicate effectively.

Responsibilities

• Conduct initial mental health assessments and provide psychotherapy.
• Document all clinical activities and maintain CMS/Medicare compliance.
• Lead and collaborate with interdisciplinary team to address residents’ needs.
• Manage your caseload, schedule, and compensation within the role.
• Support residents and families and advocate for patient-centered care.
