Generation Hospice in Ohio is seeking a compassionate Registered Nurse to join as an RN Case Manager. You will serve as the clinical point person for patients, families, and the interdisciplinary team.

Responsibilities include comprehensive assessments, care planning, symptom management, and coordination of services across home health, facilities, and hospitals. You’ll travel to patient locations and participate in on-call rotations to support end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• At least one year of clinical nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong assessment, clinical judgment, and time-management skills.
• Comfort with electronic medical records.

Responsabilités

• Perform comprehensive assessments of patients’ physical, emotional, psychosocial, and environmental needs.
• Develop and update individualized care plans with patients, families, physicians, and the interdisciplinary team.
• Provide skilled nursing care, including pain and symptom management, medication administration, wound care, and catheter care.
• Coordinate services with hospice aides, social workers, chaplains, volunteers, and therapists.
• Educate patients and caregivers on disease progression, comfort measures, medications, and what to expect at end of life.
• Present patient updates and collaborate in Interdisciplinary Group (IDG) meetings.
• Maintain accurate, timely documentation that supports hospice eligibility and complies with Medicare Conditions of Participation.
• Visit patients in homes, nursing facilities, assisted living, and hospitals across our service area.
• Take part in the on-call rotation, including some weekends and holidays.
