Job summary
Optimal Care is seeking a Registered Nurse to deliver comprehensive skilled nursing care in patients' homes or hospice settings across Clinton, Eaton, Ingham, Washtenaw, and surrounding counties. You will manage a caseload, perform assessments, and coordinate services with physicians and families.
The role emphasizes clinical independence, critical thinking, and collaboration with an interdisciplinary team to enhance patient outcomes and quality of life. Travel is daily within the service area.
Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse.
- Minimum one year experience as a nurse in a clinical care setting.
Responsibilities
- Provide skilled nursing care in patients' homes or hospice settings.
- Develop and implement individualized care plans in coordination with interdisciplinary team.
- Assess patients, monitor progress, and adjust care plans as needed.
- Coordinate services with physicians, facilities, and families.
