Qualifications

• Current state license as an RN, current driver’s license.
• One year of RN experience in a clinical care setting required; home-health experience preferred.
• Reliable transportation and valid driver’s license and auto insurance.

Responsibilities

• Administer skilled nursing care in patients’ homes for clients of all ages.
• Coordinate care with interdisciplinary team, patients, families, and referring agencies.
• Complete initial and ongoing assessments, including OASIS or other tools.
