Optimal Care is seeking a Registered Nurse to deliver skilled nursing care in patients' homes, supporting recovery and comfort in hospice or home health. You will manage a caseload, perform assessments, and coordinate with physicians and families while traveling within the service area.

Qualifications

• Graduate of an accredited nursing program (Diploma, ASN, or BSN).
• Current State License as a Registered Nurse.
• Minimum 1 year of nursing experience in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes or in hospice/home health settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate with physicians, families, and an interdisciplinary care team to ensure optimal care.
