Qualifications

• Active Oklahoma RN license required.
• Home health experience preferred; strong assessment and critical thinking.
• CPR certification required.

Responsibilities

• Develop and adjust the plan of care with the care team.
• Complete visits, OASIS assessments, and documentation.
• Coordinate with the care team to prevent anything from falling through the cracks.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients and drive real outcomes.
