Job summary

21st Century Home Health Services seeks a Home Health Physical Therapist Case Manager to evaluate and treat patients in their homes within Citrus Heights. You will develop care plans, supervise PTAs, and coordinate with physicians to ensure high-quality, patient-centered care.

Required are a CA PT license, BLS/CPR, and a CA driver's license, with home health and interdisciplinary teamwork experience preferred. A supportive, growth-oriented workplace offers competitive benefits and autonomy.

Qualifications

• Current unrestricted CA Physical Therapist License is required.
• BLS/CPR certification required.
• Valid CA Driver's License required.
• Experience in home health care and interdisciplinary teams preferred.

Responsibilities

• Evaluate and treat patients in their homes using prescribed PT programs.
• Develop detailed care plans and coordinate with PTAs per regulatory standards.
• Communicate with patients’ physicians to ensure quality level of care.
• Document progress and adjust treatment plans based on patient response.
