Job summary

VitalCaring is seeking a Home Health Physical Therapist to help patients regain mobility and independence in the comfort of their homes. This highly autonomous, relationship-driven role collaborates with nurses, physicians, and the care team to deliver individualized plans and monitor progress.

You will evaluate functional status, implement care plans, educate patients and families, and ensure safe transitions of care.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or MS).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• CPR certification is required.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
