VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, promoting independence and mobility. You will collaborate with the care team to create and adjust individualized plans, ensuring safe, effective treatment.

Formación

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately within the EMR.
