Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will assess mobility, design plans with the care team, provide hands-on therapy, and educate families while coordinating with nurses and physicians.

This role emphasizes autonomy, direct patient impact, and ongoing clinical education with opportunities for growth within a compassionate home health team.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately within the EMR; participate in team meetings.
• Supervise PTAs and provide guidance and education to caregivers.
