Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, evaluating function, planning individualized treatments, and promoting independence. Join a compassionate team focused on restoring mobility and quality of life.

You will work closely with nurses and physicians, document progress in the EMR, and educate patients and caregivers while maintaining high clinical standards and state licensure requirements.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).

Responsibilities

• Evaluate patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) and support staff.
• Document patient care and progress accurately in the EMR.
