Job summary

VitalCaring Group seeks a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will assess mobility, develop individualized plans, and provide hands-on therapy to help patients regain independence.

This role emphasizes autonomy, case coordination with nurses and physicians, and ongoing clinical education to advance your career within a compassionate team.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support transitions of care and contribute to discharge planning.
• Collaborate with nurses, therapists, and physicians for coordinated care.
• Document patient care and progress accurately within the EMR.
• Participate in case conferences and team meetings.
