Job summary

VitalCaring in the United States is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting. You will evaluate function, develop plans with physicians, and provide hands-on therapy to improve mobility and independence.

Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
• Document patient care and progress in the EMR.
