Job summary

VitalCaring Group in Texas is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings. You will assess mobility, design individualized plans, and work with physicians and care teams to help patients regain independence.

This role is highly autonomous and relationship-driven, offering flexible scheduling, opportunities for clinical education, and meaningful, lasting impact on patients and families.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Document patient care and progress in the EMR and participate in team meetings.
