Job summary

VitalCaring is seeking a Home Health Physical Therapist to provide patient-centered, in-home care. You will evaluate functional status, develop individualized plans, and deliver hands-on therapy to improve mobility and independence while educating patients and caregivers.

The role emphasizes autonomy, collaboration with the care team, and guidance to caregivers. Prior home health experience and a current PT license are preferred, with CPR and a valid driver’s license required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
