Job summary

Alternate Solutions Health Network in the United States seeks a licensed Occupational Therapist to provide comprehensive skilled care in patients' homes within a defined territory. You will educate patients and families, assess functional status, and ensure safe, compliant practice under physician orders.

You will document accurately, coordinate with agency staff, and support efforts to reduce hospitalizations through proactive visits and patient education in all phases of care.

Qualifications

• Minimum of one year of experience as an OT in an acute setting is required.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate occupational therapy judgments.

Responsibilities

• Provides skilled OT in a patient’s home as ordered by the physician.
• Evaluates functioning level using diagnostic and prognostic procedures.
• Develops and adjusts an individual plan of care.
• Documents patient progress and coordinates with supervisory staff.
• Promotes continuity of care and appropriate admissions, transfers and discharges.
• Instructs patients and families on equipment use and home exercise programs.
